The Clinical Experience Placements Handbook (pdf) provides an overview of the varying roles of the university supervisor, cooperating teacher, host school principal, and teacher candidate. It also includes suggestions and checklists that can assist you as you actively participate in this phase of the teacher preparation program. One of your responsibilities as a university supervisor is to complete the online documents identified below. These online forms help the OPS and the various program areas evaluate the clinical experience placement and make the improvements necessary to better prepare teacher candidates to meet the placement needs of our partnering PK-12 school divisions.
Instructions for accessing Assessments in myLongwood (pdf)
1. Observation and Evaluation Form for Clinical Experience Placement
a. This assessment completed a minimum of three times during each 7 ½ weeks placement for each observation visit.
b. For guidance on how to rate the teacher candidate for each indicator, please refer to the Observation and Evaluation Rubrics.
c. Please complete all blanks/fields before submitting.
2. Final Clinical Experience Evaluation
a. This assessment should be completed at the end of the 15 weeks placement.
b. For guidance on how to rate the teacher candidate for each indicator, please refer to the Conceptual Framework Rubric.
c. The confidential access number for this evaluation was provided in an email from the OPS.
3. Dispositions Assessment
a. This assessment should be completed at the end of each clinical experience placement.
b. Please complete all blanks/fields before submitting.
