Faculty-Led Programs

Please use these forms if you are going on a faculty-led study abroad or practicum program.

  • Conditions of Participation Form(pdf) - Parent signatures needed, even if you are over 18.
  • Study Abroad Approval Form (pdf) - For non-Longwood students ONLY, to be completed by home institution international studies office or faculty advisor.

Longwood Affiliated or Exchange Program

Please use the forms below if you are participating in an exchange or affiliated study abroad program.

Please note that there is a $250 administrative fee for studying abroad on exchange or affiliated programs. The fee will show up on your student account after you have registered for your program.

Students with outstanding balances owed to Longwood university will NOT be allowed to depart for a study abroad program.

    • Please note that you will have to complete applications for the exchange program or affiliated/approved non-affiliated program provider IN ADDITION TO Longwood's application forms. Be sure to check your selected program's deadlines because they may differ from Longwood's application deadlines.

  • Conditions of Participation Form (pdf)(parent signatures needed, even if you are over 18)
  • Transfer Credit Authorization(pdf) - To be completed by student and advisor. Please read the following policies:
    • **IF YOU ARE COMPLETING AN INTERNSHIP with Longwood University credit, you do not need to complete this form.
    • When you see your advisor, please be sure to bring descriptions of all the courses you plan to take while abroad.
    • Please be aware that courses MUST be approved by the department in which they are housed (for example, if you are taking an art course, it must be approved by an art advisor/department head).
    • If you need a substitution for a general education course, you must submit a petition to the Faculty Petitions Committee for approval. Getting a letter of support regarding the substitution from the department chair that houses the general education course can be helpful in this process. (This would happen if, for example, you want to take ART 150 but the international university does not offer an equivalent course. You can petition to have another class take the place of ART 150 for that particular gen ed goal.)
    • Complete and sign the form and return it with the course descriptions to Office of International Affairs. We will have it signed off by the registrar.
  • Register for State Department Safe Traveler Program 
    • Students are required to register their programs with the State Department. This involves creating an account and then entering your account to register some program details (location, dates, your passport number, contact information, etc.).
    • After you enroll, you must submit proof of registration to the Study Abroad Office. If you do not receipt a confirmation email, then take a screen shot of the page where you indicated your travel plans.
    • Upon registering for STEP, you will begin receiving routine emails from the Embassy concerning your destination. 
  • Study Abroad Approval Form (pdf)  (For non-Longwood students ONLY, to be completed by home institution international studies office or faculty advisor)
  • Study Abroad Estimated Cost of Attendance Worksheet (pdf)- This worksheet must be completed by the student and turned in as part of the study abroad application process. It will help you plan financially for your study abroad experience.

  • Sexual Misconduct Abroad (pdf) - must read and sign at the end of the document stating you have read and understand the material.

Additional Forms for Semester or Summer Exchange/Affiliated/Approved Non-Affiliated Programs

  • Flight Information Form(pdf)
  • Course Registration Form(pdf)
    • IF YOU ARE COMPLETING AN INTERNSHIP with Longwood University credit, you do not need to complete this form
    • ***PLEASE READ***When completing the course registration form, fill out the top half with all of your personal information and sign at the bottom. In the table at the bottom of the form, please ONLY write the TOTAL NUMBER OF CREDIT HOURS YOU WILL BE TAKING WHILE ABROAD. For example, if you are taking four 3-credit classes, you will write "12" in the space under "Credit Hours." Please do not fill out any of the other boxes! The Office of International Affairs will complete the rest of the table for you.
