Notetaker Accommodation Overview

What is the notetaker accommodation?
The Office Of Disability Resources provides the notetaking accommodation as an auxiliary aid for students with hearing, visual, motor limitations or those who have learning/cognitive disabilities that significantly affect the ability to take notes in class. This accommodation is disability based and approved based on student's documentation of their disability. Notetaking accommodation is designed to supplement a student’s learning process , these services are not to be used as a substitute for attending class.

Procedure: This informational sheet provides the procedures for all responsible parties.
Students Information/responsibilities: The registered ODR student will need to complete this form and give it to your instructor. Please explain to the instructor that you wish to use your notetaker accommodation that has been approved on your accommodation letter.
Instructor information: Once the instructor is shown accommodation letter with approved notetaker accommodation complete this form, and give it to selected/volunteer notetaker.
Instructor responsibilities: Explains the procedure in obtaining a notetaker in your class.
Notetaker information: the notetaker will complete this information with the instructor, the notetaker is responsible for returning all forms to the Office of Disability Resources(ODR)
Notetaker responsibilities: Explains the procedure for volunteering to do the notetaker accommodation and the expectations of the (ODR) office.
